Opioid & Substance Use Community Health Grant

The 2025 Opioid & Substance Use Community Health Grant round concluded with a total of $501,040 awarded to 11 non-profit organizations. This includes $468,540 from the opioid settlement and $32,500 leveraged from KZCF funds. Our review committee was composed of eight people including three community members with lived experience and two County Commissioners. There were seven multiyear awards with $343,540 committed for three years. In 2026, we have $113,210 available and project to fund 2-4 annual grants.

Eligibility Criteria

Organizations are eligible for funding if they are one of the following:  

  • A registered 501(c)(3) nonprofit organization. 
  • A registered faith-based organization. 
  • A public entity, such as a governmental agency or educational institution. 
  • A project fiscally sponsored by a 501(c)(3) nonprofit, faith-based organization or a public entity. 

Additionally, applicants must use the funding to: 

  • Address treatment, prevention, harm reduction and recovery outcomes. Priority for funding categories include: 50% treatment, 20% prevention, 20% harm reduction, and 10% recovery. 
  • Propose opioid remediation work that benefits residents of Kalamazoo County. 
  • Serve residents impacted by opioid use disorder. 
  • Support new or existing programs, provided that the opioid funds are not used to replace other funds already allocated. The applicant’s budget must clearly show how the awarded funds will expand opioid-related services within the organization. 
  • Complement current funding already allocated toward opioid remediation work or projects (i.e., opioid settlement funds must not supplant or replace current funding designated for such projects). 
  • Clear expansion of services for opioid remediation and related substances or mental health.
  • Propose opioid remediation work that aligns with one or more of the approved uses in Exhibit E. The full Exhibit E is accessible here.

*Funds cannot be used to reimburse the costs of activities incurred before the grant term. 

Prioritization Factors 

  • Priority will be awarded to applicants who serve populations experiencing gaps and disparities in access to services. These may include organizations serving rural communities, individuals involved in the legal system, individuals delivering culturally specific services, individuals experiencing homelessness and individuals with multiple oppressed identities (including members of the LGBTQIA+ community, individuals with a disability, and individuals commonly discriminated against due to race or ethnicity). 
  • Priority will be given to applicants who present program strategies and services that fill an urgent gap or critical need related to substance use in Kalamazoo County. 
  • Priority will be given to applicants who include evaluation metrics, such as quantifiable figures indicating the number of individuals to be served or impacted, and target success rate data and statistics. 
  • Priority will be given to applicants who reflect collaboration between multiple partners to increase efficiency, impact or reach.

How This Fund & Grants Work

  • The grant term can be annual or a three-year commitment (multiyear). 
  • Annual grants are most appropriate for a capital request, one-time event or training, and/or short-term projects or interventions. 
  • Multiyear grants are most appropriate for existing programs that are seeking to scale up or longer-term projects and interventions.  
  • At this time, there is no recommended level of request, but applicants should consider the total amount of dollars available.  
  • The opioid settlement requires that grant funds be used for opioid-related services for new or existing programs. Opioid settlement funds must not supplant (or replace) current funding already allocated towards the work or project. 
  • Funds can be used for capital needs (renovations, purchase of equipment, etc.) provided that such purchases directly relate to opioid remediation. 
  • All requests should include a 5% indirect or administrative rate. For example, a $50,000 request would include $2,500 for indirect costs. A definition of direct vs. indirect costs can be found here. 
  • Grant payments will be made as a lump sum at the beginning of the grant term.  

Other Requirements and Reporting 

  • Funded partners should expect annual site visits from KZCF staff.  
  • Funded partners should be prepared to provide bi-annual financial reporting: 
  • Report one due March 1, 2026.  
  • Report two due Sept. 1, 2026.  
  • Financial reports must include a breakdown of administrative vs. direct costs to date. 
  • Funded partners should expect to provide an annual final performance report within 30 days of the project’s end date or complete expenditure of the funds. KZCF will provide a submission link for reports. The final report should include:  
  • Data collected on outputs or outcomes. 
  • Testimonies and other case study information to demonstrate success. Photos are also acceptable, but optional. 
  • A summary of how funds were used. 
  • Narrative information on learnings and key findings.

The outcome of your request will be sent via email by December. If a grant is awarded, you’ll receive a grant agreement to sign and return.

Keep in Touch!

Funds will be sent by January, and the first round of reporting will be due on March 1, 2026. You’ll be assigned a Community Impact Officer who will be your main point of contact throughout the grant process and will send additional information as needed during the life of your grant.
